There is not enough information to determine the cause of the problem.
Maybe you have accidentally added the mark feature to the Latin script, while it should be in Hebrew.
So please provide more information which allows us to look into this. Alternatively send the project file to us for inspection.
The proofing tool shows the part where it goes wrong. The mark-to-mark feature is never applied as “vav holam” has already been substituted with “vavholam”. Then you see the anchor is not positioned correctly for “vavholam accentole”.
